Understanding Piles
Piles can be classified into two categories: internal and external. Internal piles originate at the top or rectal side of the anal canal and are located deep inside the rectum. They are not visible or palpable and typically do not cause pain. Bleeding may be the only sign of their presence. On the other hand, external piles originate at the lower end of the anal canal near the anus. They can be uncomfortable and may even prolapse, leading to a condition called thrombosis or thrombosed piles, which is characterized by external pain and swelling.
Causes of Piles
The causes of piles are relatively straightforward and are often related to bowel movements. Straining while passing large or hard stools, constipation due to inadequate fluid intake and low fiber diet, and prolonged sitting on the toilet seat are common causes. Other factors that contribute to the development of piles include chronic diarrhea, obesity, anal intercourse, pregnancy, and childbirth.
Lifestyle Management for Piles
To prevent and manage piles effectively, it is essential to focus on four pillars of lifestyle management:
1. Ahar (Diet and Nutrition)
The key aspect of diet and nutrition in preventing piles is to ensure that stools become soft enough to pass without causing strain on the anal muscles. It is recommended to consume foods rich in fiber, such as bananas, polished red rice, sprouts, and fruits with the skin. Additionally, drinking soups or buttermilk with meals can aid digestion. It is important to reduce salt intake as excess salt can lead to fluid retention and swelling in the veins. Avoid overeating, coffee, alcohol, non-vegetarian food, spicy food, and processed food. Stay hydrated by drinking around 2 to 2.5 liters of water per day. Consuming natural stool softeners like psyllium husk or castor oil can also help in complete evacuation of the bowels.
2. Bihar (Rest, Relaxation, and Sleep)
Excess pressure on the veins in the lower rectum should be avoided to prevent piles. Staying active and avoiding prolonged sitting is crucial. Taking short walks of 10 minutes two to three times a day and engaging in activities like swimming and aerobic exercises can promote relaxation and stimulate bowel function. Practicing relaxation techniques and maintaining good sleeping habits aid digestion and prevent constipation.
3. Achar (Everyday Activity)
Establishing healthy everyday activities is important in managing piles. Clearing bowels as soon as the urge is felt is essential to prevent constipation. Avoid straining during bowel movements and spending excessive time on the toilet seat. Taking a hip bath with cold water for 10 minutes or using ice pack can provide soothing relief. Yoga poses like Viparita Karani (legs-up-the-wall pose) and Sarvangasana (shoulder stand) help in relieving irritation and pain. Standing forward bend asanas, such as Padahastasana and Chakrasana, promote a healthy digestive system. Ashwini Mudra, a practice of contracting and relaxing the anal and genital muscles, strengthens the anal muscles and prevents enlargement or swelling of piles.
4. Vichar (Thought Processes)
Mental stress and worry can also contribute to the development of piles. Cultivating mindfulness, letting go of attachments to material things, and practicing gratitude can help overcome this factor. Keeping a diary of 10 positive points every day and learning and practicing meditation can bring positivity and reduce anxiety. Surrendering worries to a higher reality and trusting in the process can alleviate mental stress.

Studies have shown that with yogic management and correct lifestyle practices, piles can be completely cured, and individuals can lead a pain-free and happy life. By incorporating the four pillars of ahar, bihar, achar, and vichar into daily life, not only can the occurrence of piles be prevented, but existing piles can also be effectively managed.
Remember, prevention is always better than cure. By making these simple lifestyle changes, you can take control of your health and well-being and enjoy a life free from the discomfort of piles.
